With 3cx i normally try to keep the same internal and external the same.

I have personally had this issue on my old Cisco not with 3CX though. Cannot perform Firewall check.
Most routers, Linux based cuecker for sure will not translate the source port on the way out unless it is ifrewall in use either dynamically or port forwarded to a different internal IP address. Port forwarding not configured correctly. Just checked the nat table on the router and looks like its leaving the router on the correct port however returning on the incorrect port. View ports used by 3CX here. You need checkeer Wireshark as far as you can.
I think you are going to find you have some firweall nat going on inside your network. If one test succeeds, but two fail: It is used to determine if one-to-one port forwarding is configured which is required by the 3CX PBX.
Having compatibility issues with old phones? In your case their checker says you are. Please check your firewall configuration and try the test again. Ports may be blocked by your ISP.

I have done this test heaps of times and I always assumed the port test was simply a 3CX server probing the requisite ports from the outside.
